Yes. We complete accommodation, FMLA, and disability-related forms when clinically appropriate.
Because this service falls outside of direct treatment and insurance-covered care, form completion is not billed to insurance. Paperwork is completed as a private-pay service at the clinician’s prorated hourly rate.
We will first review the forms you are requesting to have completed and let you know whether the request falls within the scope of our services and clinical expertise. If it does, we will complete only the portions for which we are qualified and have clinically relevant information based on your evaluation or treatment.
Our role is to provide accurate, ethically sound documentation — not to guarantee approval of benefits or accommodations. If you are considering submitting paperwork, we’re happy to discuss the process during your appointment.
