Insurance Based Services

Light On Anxiety is an in-network provider with the following insurers:

Light On Anxiety is NOT a Medicare or Medicaid provider.

While the filing of insurance claims is a courtesy we extend to our clients enrolled in select insurance plans above, all charges are your responsibility from the date services are rendered. If your insurance company does not pay a claim, you accept full responsibility for payment in full for any outstanding balance.

Out of Network Reimbursement

For all other commercial insurance plans (non-Medicare or Medicaid), we are happy to submit claims electronically, on a monthly basis, to facilitate obtaining out of network payment. We recommend contacting your insurance company to learn about your out of network benefits.

All clients not utilizing in-network insurance have the right to receive a “Good Faith Estimate”. Private pay rates for clients not utilizing in-network insurance benefits are below.

All Light On Anxiety therapists, whether Master Level or PhD Level, are Certified in CBT and ERP for Anxiety, OCD and related conditions. Light On Anxiety guarantees you will be in competent, well trained and caring hands no matter who you work with.

We are happy to submit paid claims to your insurance company electronically, on a monthly basis, to facilitate obtaining out of network payment.

Some of our clients have been successful contacting their insurance companies directly and requesting a “single case exception” to be able to work with Light On Anxiety, given the difficulty of finding an in network therapists who specializes in anxiety.

Light On Anxiety is not in network with Medicare and is unable to submit out-of-network claims. By choosing to work with Light On Anxiety, you choose to completely forego the use of your health insurance benefits.

Light On Anxiety charges you for your co-pays and deductibles after claims have been processed and received from your insurance company. This ensures you are paying only what is owed. Insurance processing can take up to several weeks, so charges may be delayed from when the appointment occurred.

Light On Anxiety may be available to offer sliding scale rates for clinical services based on financial need and availability.

To learn more about sliding scale rates, contact our Care Management team at [email protected].

There is a $100 fee for any appointments cancelled or rescheduled with less than 48 hours notice. Cancellation fees are not covered by insurance and are your responsibility. If you are ill, please consider the use of telehealth to keep your appointment. Emergencies or special circumstances where notice is unable to be provided are considered on a case-by-case basis and at the discretion of Light On Anxiety.
