Light On Anxiety Payment Policy & Insurance Information

Insurance Based Services

Light On Anxiety is in network with:

  1. BCBS PPO
  2. Blue Choice PPO
  3. Aetna 

While the filing of insurance claims is a courtesy we extend to our clients enrolled in select insurance plans above, all charges are your responsibility from the date services are rendered. If your insurance company has not paid a claim within (20) days of submission, you accept full responsibility for payment in full for any outstanding balance.  

Out of Network Reimbursement

For all other insurance plans, we are happy to submit claims electronically, on a monthly basis, to facilitate obtaining out of network payment. We recommend contacting your insurance company to learn about your out of network benefits.

Private Pay Rates

All clients not utilizing in-network insurance have the right to receive a “Good Faith Estimate”. Private pay rates for clients not utilizing in-network insurance benefits are below.

  • Master level therapists: $180 for 53 minute sessions
  • Doctorate level therapists: $260 for 53 minute sessions

All Light On Anxiety therapists, whether Master Level or PhD Level, are Certified in CBT and ERP for Anxiety, OCD and related conditions. Light On Anxiety guarantees you will be in competent, well trained and caring hands no matter who you work with.

What are my mental health benefits?

It is recommended that you contact your insurance company prior to beginning treatment at Light On Anxiety in order review your specific benefits to determine if your plan requires preauthorization prior to obtaining mental health treatment as well as your copay and deductible amount.

How much will I have to pay for therapy at LOA ?

To get the most accurate information, we recommend contacting your insurance company directly, as benefits and financial responsibility may vary depending on plan type. It may be helpful to provide your insurance company with the information below to expedite the process of them informing you of your financial responsibility for treatment at LOA.

  • 90791 for intake
  • 90832 for 25 min session
  • 90834 for 40 min session
  • 90837 for 55 min session

They may also ask for the diagnosis code for what you will be obtaining treatment for.  See list below for the most frequent conditions we treat and the associated diagnosis codes.

Why don’t I see my therapist’s name on my insurance?

Light On Anxiety is in-network with BCBS PPO, Blue Choice and Aetna, even if a provider is not individually listed on your insurance’s provider finder.

I do not have BCBS or Aetna but I would like to try to use my insurance benefits for services at LOA.

We are happy to submit paid claims to your insurance company electronically, on a monthly basis, to facilitate obtaining out of network payment.  

Some of our clients have been successful contacting their insurance companies directly and requesting a “single case exception” to be able to work with Light On Anxiety, given the difficulty of finding an in network therapists who specializes in anxiety.

Is Light On Anxiety in-network with Medicare?

Light On Anxiety is not in network with Medicare.

Why do I see a charge on a date when I did not have a session?

The charges for co-pays are usually delayed from when the session occurred, due to insurance time processing (sometimes up to a month) and sometimes we receive multiple claims. We charge the patient balance once the claim is processed and received.

Do you offer sliding scale rates?

Light On Anxiety may be available to offer sliding scale rates for clinical services based on financial need and availability.

What is your cancellation policy?

There is a $100 fee for any appointments cancelled or rescheduled with less than 2 business days’ notice. Cancellation fees are not covered by insurance and are your responsibility. If you are ill, please consider the use of telehealth to keep your appointment. Emergencies or special circumstances where 2 business days are unable to be provided are considered on a case-by-case basis and at the discretion of Light On Anxiety.
